- Pay no capital gains tax on the donated securities
- Tax deduction for the fair market value of the securities on the date of transfer, regardless of initial value paid

a. Name of Donor, address, phone and/or email
b. Name of stock or mutual fund and number of shares
c. Anticipated date of transfer
d. Special instructions, if any
